In uncomplicated cases of asthma chest X-ray is normal. During acute attacks of asthma the lung fields appear hyperlucent, with generalized hyperinflation of the chest and flattening of the diaphragm.

Segmental infiltrates or atelectasis may be present as a result of mucus plugging. In chronic asthma the sternum appears bowed, with kyphosis of the thoracic spine and an increased retrosternal lucency on a lateral projection; peribronchial markings can be increased, and the hillar vessels appear enlarged.
