Allpyral DHS. A pyridine-extracted alum-precipitated crude extract of allergens used in immunotherapy. They were introduced first by Dome laboratories. Allpyral offers to the physician and patient the benefit of high top dose treatment with a greatly reduced number of injections. This has proved to be a highly successful range and is now prescribed throughout the world, in patients with allergic rhinoconjunctivitis, or allergic asthma. Prescribing information: Presentation: An Allpyral Allergen is a sterile, pyridine-extracted, alum precipitated antigen complex suspended in an aqueous solution of 0,9% saline containing 0,4% phenol as a preservative. Extracts are prepared either according to the specific sensitivities of individual patients or as standard extracts of pollens of five common grasses. Indications: Allpyral specific for the desensitisation of patients whose allergies have been determined by case history and confirmed by skin testing. Dosage and administration: Allpyral allergen extracts should be administered only by subcutaneous injections. Patients sensitive to seasonal allergens should be treated preseasonally in order to ensure that treatment is completed before the seasonal allergen becomes airborne or during the whole year, but during the pollination season it is given the one third of the maintenance dose. Patients sensitive only to perennial, i.e. non-seasonal, allergens may be treated at any time of the year. Alum-precipitated allergenic extract (Allpyral) used for immunotherapy has been associated with subcutaneous nodule formation at injection sites. Alum-precipitated allergenic extract used for immunotherapy caused extensive subcutaneous fibrosis with overlying skin changes at the site of injections. The lesions are disfiguring and appear to be permanent. This may be due in part to a type IV hypersensitivity response to the aluminum component of Allpyral extract. A CAT scan was valuable in demonstrating structural changes associated with this lesion [1].
